چکیده

6 ABSTRACT Increased intracranial pressure (ICP) secondary to severe brain injury is common. Increased ICP is commonly encountered in malignant middle cerebral artery ischemic stroke, traumatic brain injury, subarachnoid hemorrhage, and intracerebral hemorrhage. Multiple interventions – both medical and surgical – exist to manage increased ICP. Medical management is used as first-line therapy; however it is not always effective and is associated with significant risks. Decompressive hemicraniectomy is a surgical option to reduce ICP, increase cerebral compliance, and increase cerebral blood perfusion when medical management becomes insufficient. The purpose of this review is to provide an up-to-date summary of the use of decompressive hemicraniectomy for the management of refractory elevated ICP in malignant middle cerebral artery ischemic stroke, traumatic brain injury, subarachnoid hemorrhage, and intracerebral hemorrhage.

Decompressive Hemicraniectomy in Patients With Transtentorial Herniation

✔ Objective: To present the results, technique and indications of decompressive hemicraniectomy in patients with unilateral transtentorial herniation. Methods: Ten consecutive patients with intracranial hypertension refractory to medical management and transtentorial herniation were treated with decompressive hemicraniectomy over a 3-year period with the following diagnoses: malignant infarctio...

Hemicraniectomy in Malignant Middle Cerebral Artery Infarction

The concept of decompressive surgery for treatment of elevated intracranial pressure has been developed already in the beginning of the 20th century.1 The rationale of this treatment modality consists of opening of the skull and removal of a bone flap to allow the edematous brain to swell outward, thereby preventing intracranial tissue shifts and life-threatening downward herniation. The use of...

Review and recommendations on management of refractory raised intracranial pressure in aneurysmal subarachnoid hemorrhage

Intracranial hypertension is commonly encountered in poor-grade aneurysmal subarachnoid hemorrhage patients. Refractory raised intracranial pressure is associated with poor prognosis. The management of raised intracranial pressure is commonly referenced to experiences in traumatic brain injury. However, pathophysiologically, aneurysmal subarachnoid hemorrhage is different from traumatic brain i...

Decompressive hemicraniectomy for the treatment of intractable intracranial hypertension after aneurysmal subarachnoid hemorrhage.

BACKGROUND AND PURPOSE Decompressive hemicraniectomy and duroplasty (DHCD) can improve survival in patients with severe cerebral edema. We present our clinical experience with DHCD for the treatment of refractory elevated intracranial pressure (ICP) in patients with aneurysmal subarachnoid hemorrhage (aSAH). METHODS DHCD was performed in 16 patients (11 female; median age, 49.5 years) with aS...
